When you contact Winter, Chewning & Geary, LLP, we will need you to provide us with a lot of information and it is helpful if you are prepared for our first meeting. Among other things, you will need to provide us with the following information:
- Names and addresses of doctors, therapists, hospitals, and clinics who treated you for your disabling condition.
- The dates of treatment and the type of treatment you received.
- Names and dosages of medicines you are taking.
- Any medical records in your possession. (You do not need to order your own medical records if you do not already have them. We can order the records for you.)
- A list of the illnesses, injuries or conditions that limit your ability to work.
- Information regarding any prior applications for Social Security benefits that you have previously filed.
- A list of every job that you've had in the past 15 years and a brief description of the type of work performed.
